4BE IT ENACTED BY THE GENERAL ASSEMBLY OF THE STATE OF IOWA:
1 Section 1. MEDICAL EXAMINER INVESTIGATION FORM FOR DEATHS
2OF INDIVIDUALS AGE THREE AND OLDER — INFORMATION REGARDING
3IMMUNIZATIONS.
41. The medical examiner investigation form for cases
5for individuals three years of age and older shall include a
6request for information regarding the date and type of the
7decedent’s immunizations during the six-month period prior to
8death. The information provided shall include all types of
9immunizations received.
102. The department of health and human services shall
11amend the medical examiner investigation form for cases for
12individuals three years of age and older to comply with this
13section.
14EXPLANATION
15The inclusion of this explanation does not constitute agreement with
16the explanation’s substance by the members of the general assembly.
17This bill requires that a medical examiner investigation
18form for individuals three years of age and older shall include
19a request for information regarding the date and type of the
20decedent’s immunizations for the six-month period prior to
21death. The information provided shall include all types of
22immunizations received. The bill requires the department
23of health and human services to amend the medical examiner
24investigation form for individuals three years of age and older
25to comply with the bill.
