3BE IT ENACTED BY THE GENERAL ASSEMBLY OF THE STATE OF IOWA:
1 Section 1. MEDICAL EXAMINER INVESTIGATION FORM FOR
2CHILD DEATHS AGE ZERO TO THREE — INFORMATION REGARDING
3IMMUNIZATIONS.
41. The medical examiner investigation form for cases for
5infants zero to three years of age shall include a request
6for information regarding the date, brand, and type of the
7decedent’s last immunization. If the decedent received more
8than one immunization at the time of the last immunization, the
9information provided shall include all types of immunizations
10received.
112. The department of public health shall amend the medical
12examiner investigation form for cases for infants zero to three
13years of age to comply with this section.
14EXPLANATION
15The inclusion of this explanation does not constitute agreement with
16the explanation’s substance by the members of the general assembly.
17This bill requires that a medical examiner investigation
18form for infants zero to three years of age shall include
