1 3 #1. Page 7, by inserting after line 4, the
1 4 following:
1 5 " . "Medically necessary" means the provision of
1 6 a service, item, or procedure that will or is
1 7 reasonably expected to do any of the following:
1 8 a. Arrive at a correct diagnosis.
1 9 b. Prevent the onset of an illness, condition, or
1 10 disability.
1 11 c. Reduce or ameliorate the physical, mental,
1 12 developmental, or behavioral effects of an illness,
1 13 condition, injury, or disability.
1 14 d. Assist the individual to achieve or maintain
1 15 optimum functional capacity in performing age-
1 16 appropriate daily activities.
1 17 e. Minimize the deterioration of health status or
1 18 reduce or prevent suffering or pain.
1 19 Input from multiple individual sources including an
1 20 individual or an individual's family shall be
1 21 considered in determining what services are medically
1 22 necessary. The medically necessary services provided,
1 23 as well as the type of provider and setting, which may
1 24 include the individual's home, must be appropriate to
1 25 the specific health needs of the individual. A
1 26 determination of medically necessary services shall be
1 27 made on an individual basis only, and not on a class
1 28 or group basis. Determinations shall take into
1 29 account the health care practice guidelines and
1 30 standards that are issued by professionally recognized
1 31 organizations or governmental bodies."
1 32 #2. Page 8, by striking lines 8 and 9 and
1 33 inserting the following: "of medically necessary."

1 35 inserting the following: "necessary and consistent
1 36 with clinical standards of".
