  1  1    Amend the amendment, H-1309, to House File 667 as
  1  2 follows:
  1  3    #1.  Page 2, line 4, by inserting after the word
  1  4 "consumer." the following:  "Information regarding
  1  5 provider location shall identify providers located in
  1  6 the vicinity of an address selected by a consumer and
  1  7 provide contact information without listing the
  1  8 specific addresses of the providers."
