  1  1    Amend House File 2264, as passed by the House, as
  1  2 follows:
  1  3    #1.  Page 1, by inserting after line 25, the
  1  4 following:
  1  5    "Sec.    .  NEW SECTION.  146A.2A  MANDATORY
  1  6 INFORMATION – REPRODUCTIVE HEALTH SERVICES.
  1  7    An individual seeking reproductive health services
  1  8 in this state shall be provided with all of the
  1  9 following information:
  1 10    1.  The policy or practice of the hospital,
  1 11 ambulatory treatment center, or other person from whom
  1 12 the individual is seeking reproductive health services
  1 13 relating to any restrictions in the provision of
  1 14 services regarding contraceptive services including
  1 15 but not limited to emergency contraception, surgical
  1 16 sterilization, infertility treatments, and abortions.
  1 17    2.  The policy or practice of the physician or
  1 18 other licensed practitioner from whom the individual
  1 19 is seeking reproductive health services relating to
  1 20 any restrictions in the provision of services
  1 21 regarding contraceptive services including but not
  1 22 limited to emergency contraception, surgical
  1 23 sterilization, infertility treatments, and abortions."
  1 24    #2.  Title page, line l, by inserting after the
  1 25 words "relating to" the following:  "reproductive
  1 26 health including the provision of information
  1 27 regarding the reproductive health services provided by
  1 28 a specific person and including".
  1 29    #3.  By renumbering as necessary.
