  1  3    #1.  Page 2, Page 12, by inserting after the word
  1  4 "physician" the following:  "and surgeon".
  1  5    #2.  Page 2, by striking line 13 and inserting the
  1  6 following:  "148, an osteopath licensed pursuant to
  1  7 chapter 150, an osteopathic physician and surgeon
  1  8 licensed pursuant to chapter 150A, a physician
  1  9 assistant licensed pursuant to".
  1 10    #3.  Page 2, line 14, by inserting after the figure
  1 11 "148C," the following:  "a chiropractor licensed
  1 12 pursuant to chapter 151,".
  1 13    #4.  Title page, line 2, by inserting after the
  1 14 word "assistant," the following:  "a chiropractor,".
