  1  1    Section 1.  DEPARTMENT OF ELDER AFFAIRS.  There is
  1  2 appropriated from the general fund of the state to the
  1  3 department of elder affairs for the fiscal year beginning July
  1  4 1, 1997, and ending June 30, 1998, the following amount, or so
  1  5 much thereof as is necessary, to be used for the purpose
  1  6 designated:
  1  7    For the operation of a statewide case management system for
  1  8 long-term care for the frail elderly:  
  1  9  ................................................. $  1,000,000
  1 10    Funds appropriated in this subsection shall be utilized as
  1 11 follows:
  1 12    1.  To implement a mechanism to coordinate public, private,
  1 13 and volunteer long-term care services in each county using an
  1 14 integrated case management program for older adults.  The
  1 15 coordinating mechanism shall include, but not be limited to,
  1 16 coordination of services of hospitals, nursing facilities,
  1 17 department of human services, public health nursing,
  1 18 homemaker-home health programs, well-elderly clinics, and
  1 19 volunteer groups such as Alzheimer's support chapters and the
  1 20 American red cross.  The integrated case management program
  1 21 developed by the department shall apply to functionally
  1 22 dependent elderly adults at home and shall operate in
  1 23 conjunction with the long-term care coordinating unit pursuant
  1 24 to section 231.58.  The integrated case management program
  1 25 shall take a holistic approach to the development of a
  1 26 services delivery system by assessing the needs of
  1 27 individuals, identifying the continuum of services available
  1 28 in the community, monitoring service delivery and
  1 29 appropriateness through case management techniques,
  1 30 coordinating statewide policy planning, and advocating for the
  1 31 rights of long-term care consumers.  The integrated case
  1 32 management program shall be implemented throughout the
  1 33 department and the area agencies on aging.  The services
  1 34 delivery system shall use common intake procedures, assessment
  1 35 tools, and interagency working agreements for coordination
  2  1 with agencies at the local level.
  2  2    2.  To create a common database throughout the state which
  2  3 will assist policymakers in long-range planning and policy
  2  4 development relating to the case management system.
  2  5    3.  To provide training and technical assistance to
  2  6 increase the capacity of both public and private caregivers
  2  7 and advocates.
  2  8    4.  Quarterly financial impact reports for each area agency
  2  9 shall be submitted to the department and annual reports shall
  2 10 be submitted to the general assembly.  
  2 11                           EXPLANATION
  2 12    This bill makes an appropriation of $1 million for the
  2 13 fiscal year beginning July 1, 1997, and ending June 30, 1998,
  2 14 to the Iowa department of elder affairs for the operation of a
  2 15 statewide case management program for long-term care for the
  2 16 frail elderly.  The funds are to be utilized to develop a
  2 17 coordinating mechanism for public, private, and volunteer
  2 18 long-term care services under an integrated case management
  2 19 program for older adults in each county.  The integrated case
  2 20 management program shall apply to functionally dependent
  2 21 elderly adults at home and shall operate in conjunction with
  2 22 the long-term care coordinating unit.  The case management
