  1  1    Amend Senate File 2161, as passed by the Senate, as
  1  2 follows:
  1  3    #1.  Page 1, by striking line 27 and inserting the
  1  4 following:  "person shall forward any written consent
  1  5 forms may provide any relevant".
  1  6    #2.  Page 4, by inserting after line 31 the
  1  7 following:
  1  8    "c.  The department shall develop an informational
  1  9 brochure for patients who may have blood withdrawn for
  1 10 the purpose of performing an HIV test.  The
  1 11 information, at a minimum, shall include a summary of
  1 12 the patient's rights and responsibilities under the
  1 13 law."
  1 14    #3.  By renumbering, relettering, or redesignating
  1 15 and correcting internal references as necessary.
