  1  1    Amend Senate File 522 as follows:
  1  2    #1.  Page 1, line 3, by striking the word "a."
  1  3    #2.  Page 1, by striking line 4 and inserting the
  1  4 following:  "from any community based provider of,
  1  5 whether organized for pecuniary profit or not or
  1  6 whether supported by charitable or public or private
  1  7 funds, that provides treatment or services for".
  1  8    #3.  Page 1, line 5, by inserting after the word
  1  9 "health," the following:  "brain injury,".
  1 10    #4.  Page 1, by striking line 7 and inserting the
  1 11 following:  "host county in which the site of the
  1 12 provider is located unless the person continuously
  1 13 resides in the host that".
  1 14    #5.  Page 1, by striking lines 10 through 20.
  1 15    #6.  Title page, line 1, by striking the word
  1 16 "community-based".
