  1  1    Amend House File 174 as follows:
  1  2    #1.  Page 1, line 3, by striking the word "In" and
  1  3 inserting the following:
  1  4    "1.  In".
  1  5    #2.  Page 1, by inserting after line 9, the
  1  6 following:
  1  7    "2.  Supplies of application forms for a marriage
  1  8 license and marriage licenses existing prior to the
  1  9 effective date of this section may be used until
  1 10 exhausted.  In the interim, the statement required to
  1 11 be contained in the application form for a marriage
  1 12 license and marriage licenses pursuant to subsection
  1 13 1, may be attached to the form or license as an
  1 14 addendum.
  1 15    Sec. 2.  EFFECTIVE DATE.  This Act, being deemed of
  1 16 immediate importance, takes effect upon enactment."
  1 17    #3.  Title page, line 2, by inserting after the
  1 18 words "a marriage license" the following:  ", and
  1 19 providing an effective date".
