1 2 #1. Page 4, by striking lines 3 through 5 and
1 3 inserting the following: "school, and the state
1 4 school for the deaf shall submit to the department of
1 5 education by October 1, 1996, and annually for each
1 6 succeeding year in which technology funds are
1 7 received, a report that shall include, but is not
1 8 limited to, the following:
1 9 a. The amount of technology funds allocated under
1 10 this chapter.
1 11 b. The type of equipment purchased or leased
1 12 pursuant to section 295.2, subsection 3.
1 13 c. The total number of classrooms in the district.
1 14 d. The total number of functional computers used
1 15 by students on a daily basis.
1 16 e. The total number of functional computers in the
1 17 district.
1 18 f. The number of teachers receiving technology
1 19 training, and the total amount of technology and other
1 20 funds expended for the training.
1 21 g. A listing of the computer software used by each
1 22 school district, with a separate listing of computer
1 23 programs used in the classrooms by students, including
1 24 a listing of computer software, by subject matter,
1 25 used by students at each grade level, and whether the
1 26 software is available during times other than the
1 27 regular school day.
1 28 The department of education shall develop uniform
1 29 reporting forms by July 1, 1996, and shall distribute
1 30 the forms annually by July 1 to each school district
1 31 to which technology funds are allocated under this
1 32 chapter. Districts that fail to file a technology
1 33 report by October 1 of each year shall be ineligible
1 34 to receive technology funds in the current school year
1 35 and funds remaining in the district account
1 36 established for purposes of this chapter shall revert
1 37 to the technology assistance fund for allocation on a
1 38 competitive basis by the department of education
1 39 pursuant to section 295.2, subsection 2."
