  1  1    Amend House File 341, as amended, passed, and
  1  2 reprinted by the House, as follows:
  1  3    #1.  Page 5, by inserting after line 22, the
  1  4 following:
  1  5    "Sec.    .  NEW SECTION.  146B.1  TITLE.
  1  6    This chapter shall be known and may be cited as the
  1  7 "Vasectomy Waiting Period Act".
  1  8    Sec.    .  NEW SECTION.  146B.2  VASECTOMY –
  1  9 MANDATORY WAITING PERIOD.
  1 10    1.  Except in the case of a medical emergency, a
  1 11 vasectomy shall not be performed in this state unless
  1 12 the man upon whom the vasectomy is to be performed
  1 13 consents to the vasectomy at least twenty-four hours
  1 14 prior to the performance of the procedure.
  1 15    2.  The referring physician, the physician who will
  1 16 perform the vasectomy, or an agent of either physician
  1 17 shall provide information to the man, by telephone, by
  1 18 audiotape, or in person, at least twenty-four hours
  1 19 before the vasectomy regarding the method, the
  1 20 procedures commonly used, the risks commonly
  1 21 associated with each procedure, the possible
  1 22 detrimental effects of a vasectomy, and the medical
  1 23 risks commonly associated with a vasectomy.
  1 24    3.  The referring physician, the physician who will
  1 25 perform the vasectomy, or an agent of either
  1 26 physician, who provides the required information
  1 27 pursuant to subsection 2 to the man upon whom the
  1 28 vasectomy is to be performed, shall obtain and retain
  1 29 written certification from the man that the
  1 30 information was provided."
  1 31    #2.  Title page, line 1, by striking the word
  1 32 "informed".
  1 33    #3.  Title page, line 1, by inserting after the
  1 34 word "abortion" the following:  "or a vasectomy".
