  1  1    Amend Senate File 2314 as follows:
  1  2    #1.  Page 6, by striking line 26 and inserting the
  1  3 following:  "center if any of the following applies:
  1  4    a.  The person submits to the admitting official
  1  5 a".
  1  6    #2.  Page 6, by inserting after line 31 the
  1  7 following:
  1  8    "b.  The person, or if the person is a minor, the
  1  9 person's parent or legal guardian, submits an
  1 10 affidavit signed by the person or the person's parent
  1 11 or legal guardian, stating that the immunization
  1 12 conflicts with the tenets and practices of a
  1 13 recognized religious denomination of which the
  1 14 applicant is an adherent or member.
  1 15    The exemptions under this subsection do not apply
  1 16 in times of emergency or epidemic as determined by the
  1 17 state board of health and as declared by the director
  1 18 of public health."
  1 19    #3.  Page 20, line 26, by inserting after the word
  1 20 "department." the following:  "This section shall not
  1 21 be construed to require treatment of the infant's eyes
  1 22 with a prophylactic solution if the infant's parent or
  1 23 legal guardian states that such treatment conflicts
  1 24 with the tenets and practices of a recognized
  1 25 religious denomination of which the parent or legal
  1 26 guardian is an adherent or member."
  1 27    #4.  Page 20, line 31, by striking the words "a
  1 28 member" and inserting the following:  "an adherent or
  1 29 a member".
