  1  1    Amend House File 760, as amended, passed, and
  1  2 reprinted by the House, as follows:
  1  3    #1.  Page 23, by inserting after line 20, the
  1  4 following:
  1  5    "4.  By January 1, 2000, the department shall
  1  6 integrate the medical assistance and children's health
  1  7 insurance programs into a comprehensive, uniform
  1  8 system.  The uniform system shall, at a minimum,
  1  9 utilize a single application form and a single form of
  1 10 participant identification for both programs.  The
  1 11 system shall not require a face-to-face interview for
  1 12 an initial determination of eligibility for either
  1 13 program.  The department shall also eliminate the
  1 14 resource allowance test as a basis for eligibility
  1 15 under the medical assistance program."
