1 1 Amend the amendment, H-8162, to House File 2229, as
1 2 follows:
1 3 #1. Page 2, line 9, by inserting after the word
1 4 "performed" the following: "or shall be mailed to the
1 5 patient by regular mail or by restricted certified
1 6 mail, as defined in section 618.15, as requested by
1 7 the patient".
1 8 #2. Page 2, by striking line 22 and inserting the
1 9 following: "agencies available, categorized by the
1 10 type of services".
1 11 #3. Page 3, by striking lines 8 through 10 and
1 12 inserting the following: "provided. The referring".
1 13 #4. Page 3, by striking lines 19 through 28, and
1 14 inserting the following:
1 15 "5. On or before October 1, 2000, the department
1 16 shall cause the information referred to in subsection
1 17 2 to be published in printed format. The information
1 18 shall be provided in an easily comprehensible manner.
1 19 The information shall be published in a typeface large
1 20 enough to be clearly legible. The printed information
1 21 shall be available from the department at no cost,
1 22 upon request, and in an appropriate number, to any
1 23 person."
