  1  1    Amend House File 714, as passed by the House, as
  1  2 follows:
  1  3    #1.  Page 1, by inserting before line 1 the
  1  4 following:
  1  5    "Section 1.  Section 595.5, Code 1999, is amended
  1  6 to read as follows:
  1  7    595.5  SURNAME NAME ADOPTED.
  1  8    1.  A party may request indicate on the application
  1  9 for a marriage license the adoption of a name change
  1 10 to that of the other party or to some other surname
  1 11 mutually agreed upon by the parties.  The names used
  1 12 on the marriage license shall become the legal names
  1 13 of the parties to the marriage.  The marriage license
  1 14 shall contain a statement that when a name change is
  1 15 requested and affixed to the marriage license, the new
  1 16 name is the legal name of the requesting party.  If a
  1 17 party requests a name change, other than a change of
  1 18 surname to that of the other spouse or to a
  1 19 combination of the surnames of both spouses, the party
  1 20 shall request approval of the court pursuant to
  1 21 chapter 674 and shall submit to the court the
  1 22 information required by section 674.2.  Upon approval
  1 23 of the court and solemnization of the marriage, the
  1 24    2.  The county registrar shall send a certified
  1 25 copy of the return of marriage to the recorder's
  1 26 office in every county in this state where real
  1 27 property is owned by either of the parties, upon
  1 28 request of the parties.  The judge may approve the
  1 29 name change.  The new names and the immediate former
  1 30 names shall appear on the return of marriage, and the
  1 31 return of marriage shall be recorded in the
  1 32 miscellaneous records in the recorder's office.
  1 33    3.  An individual shall have only one legal name at
  1 34 any one time."
  1 35    #2.  Title page, lines 1 and 2, by striking the
  1 36 words "alternative forms of identification to be
  1 37 attached to a petition in an action for seeking a".
  1 38    #3.  Title page, line 2, by striking the word
  1 39 "change" and inserting the following:  "changes".
  1 40    #4.  By renumbering as necessary.
