  1  2 reprinted by the Senate, as follows:
  1  3    #1.  Page 21, line 13, by inserting after the word
  1  4 "assembly." the following:  "The department, in
  1  5 consultation with the board established for the child
  1  6 health care program, shall develop and utilize an
  1  7 application form, which does not exceed two pages in
  1  8 length, for coordination of the child health care
  1  9 program and the medical assistance program."
