  1  3    #1.  Page 1, by striking line 12 and inserting the
  1  4 following:
  1  5    "(4)  Physician services, including surgical and
  1  6 medical, and".
  1  7    #2.  Page 1, line 18, by inserting after the word
  1  8 "therapy" the following:  "and physical medicine".
