1 5 medical, surgical, intensive care unit, mental health,
1 6 and substance abuse services.
1 7 (2) Nursing care services including skilled
1 8 nursing facility services.
1 9 (3) Outpatient hospital services including
1 10 emergency room, surgery, lab, and x-ray services and
1 11 other services.
1 12 (4) Physician services, both surgical and medical,
1 13 including office visits, newborn care, well-baby and
1 14 well-child care, immunizations, urgent care,
1 15 specialist care, allergy testing and treatment, mental
1 16 health visits, and substance abuse visits.
1 17 (5) Ambulance services.
1 18 (6) Physical therapy.
1 19 (7) Speech therapy.
1 20 (8) Durable medical equipment.
1 21 (9) Home health care.
1 22 (10) Hospice services.
1 23 (11) Prescription drugs.
1 24 (12) Dental services including preventative
1 25 services.
1 26 (13) Medically necessary hearing services.
1 27 (14) Vision services including corrective lenses."
