  1  2    #1.  Page 1, by striking line 26 and inserting the
  1  3 following:  "shall also contain the name and address
  1  4 of the insurer or the name of the insurer and the name
  1  5 and address".
  1  6    #2.  Page 2, line 28, by striking the word
  1  7 "vehicle" and inserting the following:  "motor
  1  8 vehicle".
  1  9    #3.  Page 3, line 10, by striking the word
  1 10 "vehicle" and inserting the following:  "motor
  1 11 vehicle".
  1 12    #4.  Page 3, line 11, by striking the word
  1 13 "vehicle" and inserting the following:  "motor
