  1  1    Amend House File 2268 as follows:
  1  2    #1.  Page 1, by inserting after line 19, the
  1  3 following:
  1  4    "aa.  "Pharmaceutical care" means a comprehensive,
  1  5 patient-centered, outcomes-oriented pharmacy practice
  1  6 that requires the pharmacist to accept responsibility
  1  7 for assisting the patient in managing the patient's
  1  8 drug therapy plan.  Pharmaceutical care includes all
  1  9 of the following:
  1 10    (1)  The pharmacist working in concert with the
  1 11 patient and the patient's other health care providers
  1 12 to promote health, to prevent disease, and to assess,
  1 13 monitor, implement, and modify medication use to
  1 14 ensure that drug therapy regimens are safe and
  1 15 effective.
  1 16    (2)  The pharmacist identifying, resolving, and
  1 17 preventing potential drug therapy problems.
  1 18    (3)  The pharmacist accessing and evaluating
  1 19 patient specific information and utilizing that
  1 20 information in a plan of therapy that is understood by
  1 21 and acceptable to the patient or, when applicable, the
  1 22 patient's caregiver.
  1 23    (4)  The pharmacist, in concert with the patient
  1 24 and the patient's other healthcare providers,
  1 25 reviewing, monitoring, and modifying the drug therapy
  1 26 plan as necessary.
  1 27    #2.  Page 1, line 28, by inserting after the word
  1 28 "health" the following:  ", and reasonable
  1 29 compensation for pharmaceutical care services".
  1 30    #3.  Page 3, by inserting after line 10, the
  1 31 following:
  1 32    "i.  A provision to ensure that eligible persons
  1 33 have pharmaceutical care services, when appropriate."
  1 34    #4.  By renumbering as necessary.
