  1  2    #1.  Page 2, by striking lines 11 and 12, and
  1  3 inserting the following:  "which such the hospital is
  1  4 located or in which the principal office of the
  1  5 licensed professional is located, if applicable, prior
  1  6 to the payment of".
  1  7    #2.  Page 2, line 23, by inserting after the word
  1  8 "hospital" the following:  "or the licensed
  1  9 professional, as applicable".
