1 2 #1. Page 10, by inserting after line 23 the
1 3 following:
1 4 "14. REPORTS. An employer who conducts a drug
1 5 test pursuant to this section shall, for each fiscal
1 6 year beginning on or after July 1, 1997, file an
1 7 annual report with the division of labor services of
1 8 the department of workforce development, on forms
1 9 provided by the division, documenting separately for
1 10 each category of test the following information for
1 11 all drug or alcohol tests conducted pursuant to
1 12 subsection 7, paragraphs "a" through "g":
1 13 a. The number of drug or alcohol tests conducted
1 14 in each category.
1 15 b. The results of drug or alcohol tests conducted
1 16 in each category.
1 17 c. The number of personal injuries, and the dollar
1 18 loss for property damage, arising out of the use of
1 19 drugs or alcohol by employees.
1 20 d. The cumulative direct costs of drug or alcohol
1 21 tests in each category.
1 22 e. The cost of substance abuse evaluation and
1 23 treatment for employees in each category."
