  1  1    Section 1.  NEW SECTION.  231.61  INTEGRATED CASE-
  1  2 MANAGEMENT PROGRAM.
  1  3    The department shall assist in the establishment of  an
  1  4 integrated case-management program within the department.  The
  1  5 department shall do all of the following:
  1  6    1.  In cooperation with the long-term care coordinating
  1  7 unit, establish an interagency system for coordination of
  1  8 integrated case-management policies affecting functionally
  1  9 dependent elders living at home or in institutions.
  1 10    2.  Develop and establish in each planning and service area
  1 11 a system for coordination of long-term care services provided
  1 12 by the public, private, and volunteer sectors in an integrated
  1 13 case-management program for elders.  The providers involved
  1 14 shall include but are not limited to hospitals, nursing
  1 15 facilities, the department of human services, public health
  1 16 nursing programs, homemaker-home health programs, well-elderly
  1 17 clinics, volunteer programs including the retired senior
  1 18 volunteer program (RSVP), Alzheimer's chapters, and red cross
  1 19 chapters.
  1 20    3.  Create a common database to assist policymakers in
  1 21 long-range planning and policy development relating to the
  1 22 program.
  1 23    4.  Provide training and technical assistance to public and
  1 24 private providers and advocates.
  1 25    5.  Develop and provide to prospective applicants grant
  1 26 applications for the implementation of integrated case-
  1 27 management programs in local planning and service areas.
  1 28    Sec. 2.  NEW SECTION.  231.62  PROGRAM APPROACH.
  1 29    The integrated case-management program shall be based upon
  1 30 a holistic approach in the development of a service delivery
  1 31 system by assessing the needs of individuals, identifying the
  1 32 continuum of services available in the particular community,
  1 33 monitoring service delivery and appropriateness of services,
  1 34 coordinating statewide policy planning, and advocating for the
  1 35 rights of long-term care consumers.
  2  1    Sec. 3.  NEW SECTION.  231.63  ELEMENTS OF INTEGRATED CASE-
  2  2 MANAGEMENT PROGRAM.
  2  3    The integrated case-management program shall include all of
  2  4 the following:
  2  5    1.  Monitoring of health services and treatment through
  2  6 local interdisciplinary teams utilizing available community
  2  7 programs, whenever possible.
  2  8    2.  Provision of counseling and support programs necessary
  2  9 to enable elders to remain independent in the least
  2 10 restrictive environment possible.
  2 11    3.  Coordination with the long-term care resident's
  2 12 advocate programs established pursuant to section 231.42 to
  2 13 improve coordination between the community-based and
  2 14 institutional long-term care systems.
  2 15    Sec. 4.  Section 231.33, Code 1995, is amended by adding
  2 16 the following new subsection:
  2 17    NEW SUBSECTION.  22.  Submit quarterly reports to the
  2 18 department and annual reports by January 1 to the general
  2 19 assembly regarding the financial impact on the area agency
  2 20 resulting from the integrated case-management system
  2 21 established pursuant to sections 231.61 through 231.63.
  2 22    Sec. 5.  Section 231.58, subsection 2, paragraph a, Code
  2 23 1995, is amended to read as follows:
  2 24    a.  Develop, for legislative review, the mechanisms and
  2 25 procedures necessary to implement, utilizing current
  2 26 personnel, a case-managed system of long-term care based on a
  2 27 uniform comprehensive assessment tool.  The case-management
  2 28 system shall be implemented through coordination between the
  2 29 department and the area agencies on aging.  The system shall
  2 30 use common intake procedures, assessment tools, and
  2 31 interagency working agreements to provide coordination among
  2 32 area agencies at the local level.
  2 33    Sec. 6.  Section 231.58, subsection 2, Code 1995, is
  2 34 amended by adding the following new paragraph:
  2 35    NEW PARAGRAPH.  h.  Accept annual applications for
  3  1 development of integrated case-management programs in a
  3  2 planning and service area and make recommendations to the
  3  3 general assembly for selection and funding of particular
  3  4 programs, based upon the economic necessity of the program in
  3  5 the planning and service area.  The unit shall assist planning
  3  6 and service area applicants in completing applications.
  3  7    Sec. 7.  APPROPRIATION.  There is appropriated from the
  3  8 general fund of the state to the department of elder affairs
  3  9 for the fiscal year beginning July 1, 1995, and ending June
  3 10 30, 1996, the following amount, or so much thereof as is
  3 11 necessary, to be used for the purpose designated:
  3 12    To provide funding to planning and service areas for the
  3 13 implementation of integrated case-management programs:  
  3 14 .................................................. $  2,500,000
  3 15    Of the funds appropriated in this section not more than
  3 16 $500,000 shall be expended annually to integrated case-
  3 17 management programs.  Notwithstanding section 8.33, any
  3 18 balance remaining from the appropriation made pursuant to this
  3 19 section shall not revert to the general fund of the state but
  3 20 shall be available for expenditure during subsequent fiscal
  3 21 years for the same purpose, and shall not be transferred to
  3 22 any other program.  
  3 23                           EXPLANATION
  3 24    This bill expands the use of integrated case-management
  3 25 programs in the department of elder affairs to coordinate
  3 26 long-term care services provided for functionally dependent
  3 27 elders.  The bill provides an appropriation of $2.5 million to
  3 28 the department of elder affairs to be expended in amounts of
  3 29 no more than $500,000 annually, in the form of grants to
  3 30 applicants for implementation of integrated case-management
  3 31 programs in local planning and services areas of the state for
  3 32 the coordination of long-term care services and the
  3 33 implementation of a statewide case-management system.
