  1  1    Amend House File 667, as amended, passed, and
  1  2 reprinted by the House, as follows:
  1  3    #1.  Page 68, by inserting after line 3, the
  1  4 following:
  1  5    "Sec.    .  2003 Iowa Acts, House File 619, section
  1  6 12, subsections 2 and 3, are amended to read as
  1  7 follows:
  1  8    2.  The department of human services, in
  1  9 cooperation with the department's fiscal agent and in
  1 10 consultation with a chronic care management resource
  1 11 group consortium, shall profile medical assistance
  1 12 recipients within a select number of disease diagnosis
  1 13 categories.  The assessment shall focus on those
  1 14 diagnosis areas that present the greatest opportunity
  1 15 for impact to improved care and cost reduction.
  1 16    3.  The department of human services, in
  1 17 consultation with a chronic care management resource
  1 18 group consortium, shall conduct a chronic disease
  1 19 management pilot project for a select number of
  1 20 individuals who are participants in the medical
  1 21 assistance program.  The project shall focus on a
  1 22 select number of chronic diseases which may include
  1 23 congestive heart failure, diabetes, and asthma.  The
  1 24 initial pilot project shall be implemented by October
  1 25 1, 2003.
  1 26    Sec.    .  2003 Iowa Acts, House File 619, section
  1 27 12, subsection 4, is amended by striking the
  1 28 subsection and inserting in lieu thereof the
  1 29 following:
  1 30    4.  The department of human services may procure a
  1 31 sole source contract with a vendor to manage
  1 32 individuals with select chronic diseases following the
  1 33 conclusion of the profiling of medical assistance
  1 34 recipients.  The management of chronic diseases for
  1 35 individuals under this subsection may be coordinated
  1 36 with the pilot project established in subsection 3."
