  1  1    Amend the amendment, H-1400, to House File 668 as

  1  3    #1.  Page 1, line 35, by inserting after the figure
  1  4 "5." the following:  "A tax credit certificate shall
  1  5 contain the taxpayer's name, address, tax
  1  6 identification number, the amount of the tax credit,
  1  7 and other information required by the department of
  1  8 revenue and finance."
