1 2 #1. Page 1, line 13, by inserting after the word
1 3 "disorders" the following: ", and that the
1 4 neurobiological disorders covered for treatment under
1 5 the policy or contract shall at a minimum cover the
1 6 neurobiological disorders listed in paragraph "d" but
1 7 may also cover other illnesses commonly referred to as
1 8 neurobiological disorders".
1 9 #2. Page 1, line 27, by striking the words "at
1 10 least".
1 11 #3. Page 4, line 15, by inserting after the word
1 12 "disorders" the following: ", and that the
1 13 neurobiological disorders covered for treatment under
1 14 the policy or contract shall at a minimum cover the
1 15 neurobiological disorders listed in subparagraph (3)
1 16 but may also cover other illnesses commonly referred
