  1  2    #1.  Page 1, line 1, by inserting after the letter
  1  3 "c" the following:  ", d, e, f, g".
  1  4    #2.  Page 1, by inserting after line 5, the
  1  5 following:
  1  6    "d.  The race of the patient and the race of the
  1  7 putative father.
  1  8    e.  The age of the patient and the age of the
  1  9 putative father.
  1 10    f.  The marital status of the patient and the
  1 11 marital status of the putative father.
  1 12    g.  The educational level of the patient and the
  1 13 educational level of the putative father."
  1 14    #3.  Page 1, by inserting after line 14, the
  1 15 following:
  1 16    "Sec.    .  Section 144.29A, subsection 2,
  1 17 unnumbered paragraph 1, Code 2001, is amended to read
  1 18 as follows:
  1 19    It is the intent of the general assembly that the
  1 20 information shall be collected, reproduced, released,
  1 21 and disclosed in a manner specified by rule of the
  1 22 department, adopted pursuant to chapter 17A, which
  1 23 ensures the anonymity of the patient who experiences a
  1 24 termination of pregnancy, the putative father, the
  1 25 health care provider who identifies and diagnoses or
  1 26 induces a termination of pregnancy, and the hospital,
  1 27 clinic, or other health facility in which a
  1 28 termination of pregnancy is identified and diagnosed
  1 29 or induced.  The department may share information with
  1 30 federal public health officials for the purposes of
  1 31 securing federal funding or conducting public health
  1 32 research.  However, in sharing the information, the
  1 33 department shall not relinquish control of the
  1 34 information, and any agreement entered into by the
  1 35 department with federal public health officials to
  1 36 share information shall prohibit the use,
  1 37 reproduction, release, or disclosure of the
  1 38 information by federal public health officials in a
  1 39 manner which violates this section.  The department
  1 40 shall publish, annually, a demographic summary of the
  1 41 information obtained pursuant to this section, except
  1 42 that the department shall not reproduce, release, or
  1 43 disclose any information obtained pursuant to this
  1 44 section which reveals the identity of any patient,
  1 45 health care provider, hospital, clinic, or other
  1 46 health facility, and shall ensure anonymity in the
  1 47 following ways:"
