PAG LIN 1 1 Amend House File 382 as follows: 1 2 #1. Page 1, by inserting before line 1 the 1 3 following: 1 4 "Section 1. Section 249A.3, Code 2001, is amended 1 5 by adding the following new subsection: 1 6 NEW SUBSECTION. 4A. The department shall 1 7 establish presumptive eligibility under the medical 1 8 assistance program for a child who is under nineteen 1 9 years of age. A determination of presumptive 1 10 eligibility shall be made only once in a twelve-month 1 11 period. 1 12 Sec. 2. NEW SECTION. 280.7A HEALTHY AND WELL 1 13 KIDS IN IOWA (HAWK-I) INFORMATION. 1 14 The board of directors of each school district 1 15 shall require the school district to provide healthy 1 16 and well kids in Iowa program information and 1 17 application forms to all students at the time of 1 18 school registration. The application forms shall be 1 19 provided by the healthy and well kids in Iowa program 1 20 pursuant to section 514I.7, subsection 2, paragraph 1 21 "h"." 1 22 #2. Page 1, by striking lines 5 through 9 and 1 23 inserting the following: 1 24 "a. Develop a program application form not to 1 25 exceed two pages in length, which is consistent with 1 26 the rules of the board, which is easy to understand, 1 27 complete, and concise, and which, to the greatest 1 28 extent possible, coordinates with the medical 1 29 assistance program. The program application form 1 30 shall provide a space in which the applicant may 1 31 provide the name and contact information of any person 1 32 who assisted the applicant in applying for the program 1 33 and may authorize the administrative contractor to 1 34 notify the person regarding the outcome of the 1 35 application with the sole purpose of the notification 1 36 being that the person will continue to assist the 1 37 applicant in obtaining and retaining health insurance 1 38 coverage." 1 39 #3. Page 2, by inserting after line 19 the 1 40 following: 1 41 "Sec. . Section 514I.5, subsection 8, paragraph 1 42 e, Code 2001, is amended by adding the following new 1 43 subparagraph: 1 44 NEW SUBPARAGRAPH. (15) (a) Care coordination. 1 45 For the purposes of this subparagraph subdivision, 1 46 "care coordination" means coordinating the provision 1 47 of services to children and families to assure that 1 48 the children receive health care services by promoting 1 49 the coordination of social supports and medical 1 50 services across organizations and providers. Care 2 1 coordination may include but is not limited to 2 2 educating families about the services provided under 2 3 the family's health insurance coverage plan; assisting 2 4 families in selecting providers; assisting families 2 5 with scheduling of health care appointments, 2 6 transportation to attend health care appointments, and 2 7 translation during health care appointments; and 2 8 assisting families in accessing community support 2 9 services. 2 10 (b) Dental services including the coverage of 2 11 partial dentures and dentures, with an annual coverage 2 12 maximum of one thousand five hundred dollars. 2 13 (c) Mental health and substance abuse benefits 2 14 including coverage of Axis I diagnoses as specified in 2 15 the diagnostic and statistical manual of mental 2 16 disorders; coverage of the full continuum of treatment 2 17 services; provision of adequate provider panels; use 2 18 of admission, discharge, continued stay, and placement 2 19 criteria specific to children and adolescents; and the 2 20 use of Iowa juvenile placement criteria for substance 2 21 abuse services. 2 22 (d) Medically necessary nutrition services 2 23 provided by a licensed dietician based upon a 2 24 physician referral. 2 25 (e) Physical and occupational therapy services." 2 26 #4. Page 3, by inserting after line 35 the 2 27 following: 2 28 "Sec. . Section 514I.8, subsection 2, paragraph 2 29 c, Code 2001, is amended to read as follows: 2 30 c. Is a member of a family whose income does not 2 31 exceed two hundred percent of the federal poverty 2 32 level, as defined in 42 U.S.C. } 9902(2), including 2 33 any revision required by such section. Determination 2 34 of an applicant's income shall be made on the basis of 2 35 the applicant's self-declaration of income." 2 36 #5. Page 4, by inserting after line 7 the 2 37 following: 2 38 "Sec. . Section 514I.8, subsection 3, Code 2 39 2001, is amended to read as follows: 2 40 3. In accordance with the rules adopted by the 2 41 board, a childmayshall be determined to be 2 42 presumptively eligible for the program pending a final 2 43 eligibility determination. Following final 2 44 determination of eligibility by the administrative 2 45 contractor, a child shall be eligible for a twelve- 2 46 month period. At the end of the twelve-month period, 2 47 the administrative contractor shall conduct a review 2 48 of the circumstances of the eligible child's family to 2 49 establish eligibility and cost sharing for the 2 50 subsequent twelve-month period." 3 1 #6.
