  1  3 and inserting the following:  "may".
  1  4    #2.  Page 3, line 32, by striking the word
  1  5 "required" and inserting the following:  "was
  1  6 provided".
  1  7    #3.  Page 3, line 33, by striking the words "was
  1  8 provided" and inserting the following:  "upon the
  1  9 woman's request".
  1 10    #4.  Page 4, by striking lines 2 through 4, and
  1 11 inserting the following:  "the certification to the
  1 12 woman."
  1 13    #5.  Page 4, line 34, by striking the word
  1 14 "required" and inserting the following:  "requested by
  1 15 the woman".
