  1  1    Amend House File 116 as follows:
  1  2    #1.  Page 1, by inserting before line 1, the
  1  3 following:
  1  4    "Section 1.  Section 144.29A, subsection 1,
  1  5 unnumbered paragraph 1, Code 2001, is amended to read
  1  6 as follows:
  1  7    A health care provider who initially identifies and
  1  8 diagnoses a spontaneous termination of pregnancy or
  1  9 who induces a termination of pregnancy shall file with
  1 10 the department a report for each termination within
  1 11 thirty days of the occurrence.  The health care
  1 12 provider shall make a good faith effort to obtain all
  1 13 of the following information that is available with
  1 14 respect to each termination:"
  1 15    2.  Page 1, by inserting before line 8, the
  1 16 following:
  1 17    "Sec.    .  Section 144.29A, subsection 1,
  1 18 paragraph h, Code 2001, is amended to read as follows:
  1 19    h.  The number of previous pregnancies, live
  1 20 births, and spontaneous or induced terminations of
  1 21 pregnancies."
  1 22    #3.  Page 1, by inserting after line 14, the
  1 23 following:
  1 24    "Sec.   .  Section 144.29A, subsection 9, Code
  1 25 2001, is amended by striking the subsection."
  1 26    #4.  By renumbering as necessary.
