  1  1    Amend the amendment, S-3217, to House File 664, as
  1  2 amended, passed, and reprinted by the House, as
  1  3 follows:
  1  4    #1.  Page 2, line 9, by striking the word
  1  5 "thirteen" and inserting the following:  "fifteen".
  1  6    #2.  Page 2, by inserting after line 25 the
  1  7 following:
  1  8    "Sec.    .  Section 331.438, subsection 4,
  1  9 paragraph b, subparagraph (2), Code 1999, is amended
  1 10 to read as follows:
  1 11    (2)  The committee shall include one member two
  1 12 members nominated by service providers, one member
  1 13 nominated by service advocates, and consumers one
  1 14 member who is a service consumer, and one member
  1 15 nominated by the state's council of the association of
  1 16 federal, state, county, and municipal employees, with
  1 17 these members appointed by the governor."
  1 18    #3.  Page 3, line 16, by striking the word "four"
  1 19 and inserting the following:  "five".
  1 20    #4.  By renumbering as necessary.
