1 3 #1. Page 1, line 4, by striking the word "a" and
1 4 inserting the following: "an individual or".
1 5 #2. Page 1, by striking line 6 and inserting the
1 6 following: "medical expenses shall make available a
1 7 coverage provision for contraceptive benefits as
1 8 provided in this section. Such policy or contract, if
1 9 the coverage provision for contraceptive benefits is
1 10 accepted by the policyholder, shall not do either of
1 11 the following:"
1 12 #3. Page 1, by inserting after line 17 the
1 13 following:
1 14 "1A. A policy or contract subject to subsection 1
1 15 shall not include a provision for contraceptive
1 16 benefits as provided under this section, unless such
1 17 provision is expressly accepted by the policyholder,
1 18 as evidenced by written acceptance signed by the
1 19 policyholder."
1 20 #4. Page 1, line 18, by striking the word "a" and
1 21 inserting the following: "an individual or".
1 22 #5. Page 3, by striking lines 6 through 14.
