1 3 #1. Page 1, by striking lines 7 and 8 and
1 4 inserting the following: "the occurrence. The health
1 5 care provider shall make a good faith effort to obtain
1 6 all of the following information that is available
1 7 with respect to each".
1 8 #2. Page 1, by striking lines 10 through 28 and
1 9 inserting the following:
1 10 "a. The confidential health care provider code as
1 11 assigned by the department.
1 12 b. The report tracking number.
1 13 c. The maternal health services region of the Iowa
1 14 department of public health, as designated as of July
1 15 1, 1997, in which the patient resides.
1 16 d. The race of the patient.
1 17 e. The age of the patient.
1 18 f. The marital status of the patient.
1 19 g. The educational level of the patient.
1 20 h. The number of previous pregnancies, live
1 21 births, and spontaneous or induced termination of
1 22 pregnancies.
1 23 i. The month and year in which the termination
1 24 occurred.
1 25 j. The number of weeks since the patient's last
1 26 menstrual period and a clinical estimate of
1 27 gestation."
1 28 #3. Page 3, line 2, by striking the word "avoids"
1 29 and inserting the following: "prevents".
1 30 #4. Page 4, by inserting after line 5 the
1 31 following:
1 32 "___. For the purposes of this section,
1 33 "spontaneous termination of pregnancy" means the
1 34 occurrence of an unintended termination of pregnancy
1 35 at any time during the period from conception to
1 36 twenty weeks gestation and which is not a spontaneous
1 37 termination of pregnancy at any time during the period
1 38 from twenty weeks or greater which is reported to the
1 39 department as a fetal death under this chapter."
1 40 #5. Page 4, line 8, by striking the word
1 41 "Violates" and inserting the following: "Knowingly
1 42 violates".
1 43 #6. By renumbering, relettering, or redesignating
1 44 and correcting internal references as necessary.
