PAG LIN 1 1 Section 1. Section 321.182, subsection 1, Code 1997, is 1 2 amended to read as follows: 1 3 1. Make application on a form provided by the department 1 4 which shall include the applicant's full name, signature, 1 5 current mailing address, current residential address, date of 1 6 birth,social security number,and physical description 1 7 including sex, height, and eye color. The application may 1 8 contain other information the department may require by rule, 1 9 except that an applicant's social security number shall not be 1 10 required to be included on the form unless the applicant is 1 11 applying for a commercial driver's license. 1 12 Sec. 2. Section 321.189, subsection 2, paragraph a, Code 1 13 Supplement 1997, is amended to read as follows: 1 14 a. Appearing on the driver's license shall be a 1 15 distinguishing number assigned to the licensee, which shall 1 16 not be the licensee's social security number except as 1 17 provided in paragraph "b"; the licensee's full name, date of 1 18 birth, sex, and residence address; a colored photograph; a 1 19 physical description of the licensee; the name of the state; 1 20 the dates of issuance and expiration; and the usual signature 1 21 of the licensee. The license shall identify the class of 1 22 vehicle the licensee may operate and the applicable 1 23 endorsements and restrictions which the department shall 1 24 require by rule. 1 25 Sec. 3. Section 321.189, subsection 2, paragraph c, Code 1 26 Supplement 1997, is amended by striking the paragraph. 1 27 Sec. 4. Section 321L.2, subsection 1, paragraph a, 1 28 unnumbered paragraph 1, Code Supplement 1997, is amended to 1 29 read as follows: 1 30 A resident of the state with a disability desiring a 1 31 persons with disabilities parking permit shall apply to the 1 32 department upon an application form furnished by the 1 33 department providing the applicant's name, address, and date 1 34 of birth,and social security numberand shall also provide a 1 35 statement from a physician licensed under chapter 148, 149, 2 1 150, or 150A, a physician assistant licensed under chapter 2 2 148C, an advanced registered nurse practitioner licensed under 2 3 chapter 152, or a chiropractor licensed under chapter 151, or 2 4 a physician, physician assistant, nurse practitioner, or 2 5 chiropractor licensed to practice in a contiguous state, 2 6 written on the physician's, physician assistant's, nurse 2 7 practitioner's, or chiropractor's stationery, stating the 2 8 nature of the applicant's disability and such additional 2 9 information as required by rules adopted by the department 2 10 under section 321L.8. If the person is applying for a 2 11 temporary persons with disabilities parking permit, the 2 12 physician's, physician assistant's, nurse practitioner's, or 2 13 chiropractor's statement shall state the period of time during 2 14 which the person is expected to be disabled and the period of 2 15 time for which the permit should be issued, not to exceed six 2 16 months. 2 17 Sec. 5. APPLICABILITY. The state department of 2 18 transportation may continue to use a social security number on 2 19 a license or persons with disabilities parking permit only 2 20 until the license or permit is renewed on or after the 2 21 effective date of this Act. This section does not apply to 2 22 commercial driver's licenses. 2 23 EXPLANATION 2 24 This bill prohibits the state department of transportation 2 25 from using social security numbers as the distinguishing 2 26 number appearing on driver's licenses and persons with 2 27 disabilities parking permits. The prohibition does not apply 2 28 to commercial driver's licenses. Social security numbers 2 29 would need to be deleted from such licenses and permits upon 2 30 renewal on or after the effective date of the bill. 2 31 LSB 3364HH 77 2 32 jm/jl/8
