  1  1    Amend Senate File 128, as amended, passed, and
  1  2 reprinted by the Senate, as follows:
  1  3    #1.  Page 1, by striking lines 10 through 28 and
  1  4 inserting the following:
  1  5    "a.  The confidential health care provider code as
  1  6 assigned by the department.
  1  7    b.  The report tracking number.
  1  8    c.  The region of the department of human services
  1  9 in which the patient resides.
  1 10    d.  The race of the patient.
  1 11    e.  The age of the patient.
  1 12    f.  The marital status of the patient.
  1 13    g.  The number of previous pregnancies, live
  1 14 births, and spontaneous or induced termination of
  1 15 pregnancies.
  1 16    h.  The month and year in which the termination
  1 17 occurred.
  1 18    i.  The number of weeks since the patient's last
  1 19 menstrual period and a clinical estimate of
  1 20 gestation."
