  1  1    Amend Senate File 2389 as follows:
  1  2    #1.  Page 1, line 4, by striking the word
  1  3 "Notwithstanding" and inserting the following:  "1.
  1  4 Notwithstanding".
  1  5    #2.  By striking page 1, line 33, through page 2,
  1  6 line 7 and inserting the following:
  1  7    "2.  Notwithstanding section 514C.6, a managed care
  1  8 health plan or indemnity plan with a limited provider
  1  9 network shall accept as locations for provision of
  1 10 services, a rural hospital if the hospital agrees to
  1 11 the conditions of the provider contract and the fees
  1 12 established under the contract for specific services
  1 13 as required by the managed care health plan or
  1 14 indemnity plan with a limited provider network.
  1 15    3.  For the purposes of this section:
  1 16    a.  "Managed care health plan or indemnity plan
  1 17 with a limited provider network" means a health
  1 18 maintenance organization, organized delivery system,
  1 19 exclusive provider organization, point of service
  1 20 plan, standard indemnity insurance plan, or any
  1 21 similar plan providing for health care services.
  1 22    b.  "Physician" means physician as defined in
  1 23 section 135.1 and licensed under chapter 148, 150A, or
  1 24 151.
  1 25    c.  "Rural county" means a county with a population
  1 26 of thirty-five thousand or less.
  1 27    d.  "Rural hospital" means a licensed hospital
  1 28 which is located in a rural county and which is the
  1 29 only hospital licensed in that county."
  1 30    #3.  Title page, line 3, by inserting after the
  1 31 word "physicians" the following:  "and other
  1 32 providers".
