  1  1    Amend House File 522 as follows:
  1  2    #1.  Page 2, by striking lines 15 and 16 and
  1  3 inserting the following:
  1  4    "b.  The department shall enter the data from any
  1  5 report of termination submitted, within thirty days of
  1  6 receipt of the report, and shall immediately destroy
  1  7 the report following entry of the data."
