  1  1    Amend the amendment, H-3339, to House File 336 as
  1  2 follows:
  1  3    #1.  Page 9, line 33, by striking the word and
  1  4 figures "October 15, 1995," and inserting the
  1  5 following:  "January 5, 1996,".
  1  6    #2.  Page 9, line 34, by striking the word
  1  7 "January" and inserting the following:  "March".
  1  8    #3.  Page 9, line 50, by inserting after the word
  1  9 "services." the following:  "Initially, this part of
  1 10 the plan shall be submitted within nine months of the
  1 11 date by which the department approves a managed care
  1 12 contractor."
  1 13    #4.  Page 10, line 2, by striking the word "six"
  1 14 and inserting the following:  "twelve".
