1 5 #2. Page 1, line 37, by inserting after the word
1 6 "abortion." the following: "The video shall be
1 7 updated, annually."

1 15 source. A licensed physician shall not perform an
1 16 abortion on a pregnant minor prior to obtaining the
1 17 completed certification form from the pregnant minor
1 18 or prior to making a notation in the pregnant minor's
1 19 medical record of the prior offering by another
1 20 source."
