3BE IT ENACTED BY THE GENERAL ASSEMBLY OF THE STATE OF IOWA:
1   Section 1.  DEATH CERTIFICATE FORM — INFORMATION REGARDING
2COVID-19 VACCINATION — DOCUMENTING AND REPORTING.
  The
3department of public health shall amend the death certificate
4form to include a request for information regarding the date
5of the decedent’s most recent COVID-19 vaccination. Based on
6the information received, the bureau of health statistics shall
7compile data from certified death certificates documenting
8the deaths of individuals who received a COVID-19 vaccination
9within seven days of the individual’s death and shall report
10the data in the vital statistics of Iowa annual report.
11EXPLANATION
12The inclusion of this explanation does not constitute agreement with
13the explanation’s substance by the members of the general assembly.
   14This bill requires that the department of public health
15amend the death certificate form to include a request for
16information regarding the date of the decedent’s most recent
17COVID-19 vaccination. Based on the information received,
18the bureau of health statistics shall compile data from
19the certified death certificates documenting the deaths of
20individuals who received a COVID-19 vaccination within seven
21days of the individual’s death and shall report the data in the
22vital statistics of Iowa annual report.
