3BE IT ENACTED BY THE GENERAL ASSEMBLY OF THE STATE OF IOWA:
1   Section 1.  MEDICAL EXAMINER INVESTIGATION FORM FOR
2CHILD DEATHS AGE ZERO TO THREE — INFORMATION REGARDING
3IMMUNIZATIONS.
   41.  The medical examiner investigation form for cases for
5infants zero to three years of age shall include a request
6for information regarding the date and type of the decedent’s
7last immunization. If the decedent received more than
8one immunization at the time of the last immunization, the
9information provided shall include all types of immunizations
10received.
   112.  The department of public health shall adopt rules
12pursuant to chapter 17A to amend the medical examiner
13investigation form for cases for infants zero to three years of
14age to comply with this section.
15EXPLANATION
16The inclusion of this explanation does not constitute agreement with
17the explanation’s substance by the members of the general assembly.
   18This bill requires that a medical examiner investigation
19form for infants zero to three years of age shall include a
20request for information regarding the date and type of the
21decedent’s last immunization. If the decedent received more
22than one immunization at the time of the last immunization, the
23information provided shall include all types of immunizations
24received. The bill requires the department of public health
25to adopt administrative rules to amend the medical examiner
26investigation form for infants zero to three years of age to
27comply with the bill.
