3BE IT ENACTED BY THE GENERAL ASSEMBLY OF THE STATE OF IOWA:
1 Section 1. INFANT CERTIFICATE OF DEATH FORM — INFORMATION
2REGARDING IMMUNIZATIONS.
31. The certificate of death form for medical examiner
4cases for infants zero to three years of age shall include
5a request for information regarding the date and type of the
6decedent’s last immunization. If the decedent received more
7than one immunization at the time of the last immunization, the
8information provided shall include all types of immunizations
9received.
102. The department of public health shall adopt rules
11pursuant to chapter 17A to amend the certificate of death form
12for medical examiner cases for infants zero to three years of
13age to comply with this section.
14EXPLANATION
15The inclusion of this explanation does not constitute agreement with
16the explanation’s substance by the members of the general assembly.
17This bill requires that a certificate of death form for
18medical examiner cases for infants zero to three years of age
19shall include a request for information regarding the date and
20type of the decedent’s last immunization. If the decedent
21received more than one immunization at the time of the last
22immunization, the information provided shall include all types
23of immunizations received. The bill requires the department
24of public health to adopt administrative rules to amend the
25certificate of death form for medical examiner cases for
26infants zero to three years of age to comply with the bill.
