1.
The
medical
examiner
investigation
form
for
cases
4
for
individuals
three
years
of
age
and
older
shall
include
a
5
request
for
information
regarding
the
date
and
type
of
the
6
decedent’s
immunizations
during
the
six-month
period
prior
to
7
death.
The
information
provided
shall
include
all
types
of
8
immunizations
received.

The
department
of
health
and
human
services
shall
10
amend
the
medical
examiner
investigation
form
for
cases
for
11
individuals
three
years
of
age
and
older
to
comply
with
this
12
section.

EXPLANATION
14
The
inclusion
of
this
explanation
does
not
constitute
agreement
with
15
the
explanation’s
substance
by
the
members
of
the
general
assembly.

This
bill
requires
that
a
medical
examiner
investigation
17
form
for
individuals
three
years
of
age
and
older
shall
include
18
a
request
for
information
regarding
the
date
and
type
of
the
19

immunizations
for
the
six-month
period

The
information

shall
include
