S.F. 2033 Section 1. NEW SECTION . 135.39F Providers of COVID-19 1 vaccine —— provision of product-specific fact sheet. 2 1. A provider who has enrolled in the centers for disease 3 control and prevention of the United States department of 4 health and human services COVID-19 vaccination program to 5 administer a COVID-19 vaccine shall provide to each patient 6 or their caregiver prior to administration of the vaccine 7 the most recent product-specific COVID-19 vaccine fact sheet 8 for recipients and caregivers developed by the manufacturer 9 of the vaccine and authorized by the United States food and 10 drug administration. A provider administering the vaccine 11 shall document provision of the fact sheet to the patient or 12 their caregiver in the provider’s medical record system and 13 shall report administration of the vaccine to the immunization 14 information system as required by the centers for disease 15 control and prevention of the United States department of 16 health and human services. 17 2. For the purposes of this section, “COVID-19” means the 18 same as defined in section 686D.2. 19 EXPLANATION 20 The inclusion of this explanation does not constitute agreement with 21 the explanation’s substance by the members of the general assembly. 22 This bill requires a provider who has enrolled in the 23 centers for disease control and prevention of the United 24 States department of health and human services (CDC) COVID-19 25 vaccination program to administer a COVID-19 vaccine to provide 26 to each patient or their caregiver prior to administration 27 of the vaccine the most recent product-specific COVID-19 28 vaccine fact sheet for recipients and caregivers.
