1.
DEATH
CERTIFICATE
FORM

INFORMATION
REGARDING
1
COVID-19
VACCINATION

DOCUMENTING
AND
REPORTING.
The
2
department
of
public
health
shall
amend
the
death
certificate
3
form
to
include
a
request
for
information
regarding
the
date
4
of
the
decedent’s
most
recent
COVID-19
vaccination.
Based
on
5
the
information
received,
the
bureau
of
health
statistics
shall
6
compile
data
from
certified
death
certificates
documenting
7
the
deaths
of
individuals
who
received
a
COVID-19
vaccination
8
within
seven
days
of
the
individual’s
death
and
shall
report
9
the
data
in
the
vital
statistics
of
Iowa
annual
report.

The
inclusion
of
this
explanation
does
not
constitute
agreement
with

the
explanation’s
substance
by
the
members
of
the
general
assembly.

This
bill
requires
that
the
department
of
public
health

the
death
certificate
form
to
include
a
request
for

information
regarding
the
date
of
the
decedent’s
most
recent

COVID-19
vaccination.
Based
on
the
information
received,

the
bureau
of
health
statistics
shall
compile
data
from

the
certified
death
certificates

the
deaths
of

individuals
who
received
a
COVID-19
vaccination
within

days
of
the
individual’s
death
and
shall
report
the
data
in
the

statistics
of
