1.
The
medical
examiner
investigation
form
for
cases
for
4
infants
zero
to
three
years
of
age
shall
include
a
request
5
for
information
regarding
the
date
and
type
of
the
decedent’s
6
last
immunization.
If
the
decedent
received
more
than
7
one
immunization
at
the
time
of
the
last
immunization,
the
8
information
provided
shall
include
all
types
of
immunizations
9
received.

2.
The
department
of
public
health
shall
adopt
rules
11
pursuant
to
chapter
17A
to
amend
the
medical
examiner
12
investigation
form
for
cases
for
infants
zero
to
three
years
of
13
age
to
comply
with
this
section.

EXPLANATION

The
inclusion
of
this
explanation
does
not
constitute
agreement
with
16
the
explanation’s
substance
by
the
members
of
the
general
assembly.

This
bill
requires
that
a
medical
examiner
investigation
18
form
for
infants
zero
to
three
years
of
age
shall
include
a
19
request
for
information
regarding
the
date
and
type
of
the
20
decedent’s
last
immunization.
If
the
decedent
received
more
21
than
one
immunization
at
the
time
of
the
last
immunization,
the
22
information
provided
shall
include
all
types
of
immunizations
23
received.
The
bill
requires
the
department
of
public
health

to
adopt
administrative
rules
to
amend
the
medical
examiner

investigation
form
for
infants
zero
to
three
years
of
age
to

comply
with
the
bill.
