H.F. 631 Section 1. MEDICAL EXAMINER INVESTIGATION FORM FOR 1 CHILD DEATHS AGE ZERO TO THREE —— INFORMATION REGARDING 2 IMMUNIZATIONS. 3 1. The medical examiner investigation form for cases for 4 infants zero to three years of age shall include a request 5 for information regarding the date, brand, and type of the 6 decedent’s last immunization. If the decedent received more 7 than one immunization at the time of the last immunization, the 8 information provided shall include all types of immunizations 9 received. 10 2. The department of public health shall amend the medical 11 examiner investigation form for cases for infants zero to three 12 years of age to comply with this section. 13 EXPLANATION 14 The inclusion of this explanation does not constitute agreement with 15 the explanation’s substance by the members of the general assembly. 16 This bill requires that a medical examiner investigation 17 form for infants zero to three years of age shall include 18 a request for information regarding the date, brand, and 19 type of the decedent’s last immunization. If the decedent 20 received more than one immunization at the time of the last 21 immunization, the information provided shall include all types 22 of immunizations received.
