1.
Section
139A.8,
subsection
4,
paragraph
a,
1
subparagraph
(2),
Code
2017,
is
amended
to
read
as
follows:

(2)
The
applicant,
or
if
the
applicant
is
a
minor,
the

applicant’s
parent
or
legal
guardian,
submits
an
affidavit
4
signed
by
both
the
applicant,
or
if
the
applicant
is
a
minor,
5
the
applicant’s
parent
or
legal
guardian,
and
the
leader
of
the
6
local
congregation
of
a
recognized
religious
denomination
of
7
which
the
applicant
is
an
adherent
or
member,
stating
that
the
8
immunization
conflicts
with
the
tenets
and
practices
of
a
the
9
recognized
religious
denomination
of
to
which
the
applicant
10
is
an
adherent
or
member
local
congregation
belongs,
and
11
that
the
applicant
is
a
member
in
good
standing
of
the
local
12
congregation
.

inclusion

this
explanation
does

constitute
agreement
with

the
members

general

This
bill
relates
to
the
obtaining
of
a
religious
exemption
17
from
the
required
immunization
of
a
person
enrolling
in
any
18
elementary
or
secondary
school
or
a
licensed
child
care
center.

Currently,
an
applicant,
or
if
the
applicant
is
a
minor,
the
20
applicant’s
parent
or
legal
guardian,
must
submit
an
affidavit
21
signed
by
the
applicant,
or
if
the
applicant
is
a
minor,
the
22
applicant’s
parent
or
legal
guardian,
stating
that
the
required
23
immunization
conflicts
with
the
tenets
and
practices
of
a
24
recognized
religious
denomination
of
which
the
applicant
is
an
25
adherent
or
member
in
order
to
be
exempt
from
the
immunization
26
requirement.
The
bill
amends
this
provision
to
also
require
27
that,
in
addition
to
the
applicant,
or
the
applicant’s
parent
28
or
legal
guardian,
the
leader
of
the
local
congregation
of
a
29
recognized
religious
denomination
of
which
the
applicant
is
30
an
adherent
or
member,
sign
the
affidavit
stating
that
the
31
required
immunization
conflicts
with
the
tenets
and
practices
32
of
the
recognized
religious
denomination
to
which
the
local
33
congregation
belongs
and
that
the
applicant
is
a
member
in
good
34
standing
of
the
local
congregation.
