SUPPORTS.
It
is
the
intent
of
the
general
assembly
that
a
2
patient
discharged
from
a
licensed
hospital
to
the
patient’s
3
residence
be
provided
with
appropriate
discharge
planning
to
4
ensure
that
necessary
post-hospital
services
and
supports
are
5
in
place
for
a
successful
transition
to
the
patient’s
residence
6
and
for
implementation
of
the
discharge
plan.

EXPLANATION

The
inclusion
of
this
explanation
does
not
constitute
agreement
with

the
explanation’s
substance
by
the
members
of
the
general
assembly.

This
bill
provides
that
it
is
the
intent
of
the
general

assembly
that
a
patient
discharged
from
a
licensed
hospital
to

the
patient’s
residence
be
provided
with
appropriate
discharge

planning
to
ensure
that
necessary
post-hospital
services
and

supports
are

place
